SA tags in email body?

Strange little issue where SA checks email generated by the server or formail scripts and adds it's tags to the body of the email instead of the headers? Not sure how to trouble shoot?

BTW this only occurs on server generated email and not manually/human sent emails.

Example:

Might be the way that your script is generating the email
