Don't assume I'm correct, but I think I've seen a lot of this sort of thing on our servers too. At least when I was looking at something the other day I found trying to trace a chain of events harder than I'd like because some IDs or other were being reused.
They were only getting re-used if the original was done with though - as far as I could tell.
